We have an exciting opportunity for an Assessor to join our Aspire4Life team on a full time basis as a Multi Program Assessor. This is an office-based role which will see you support the daily assessment and coordination or operations for our Aspire4Life programs. You will work between the Veterans Home Care program (VHC), and Regional Assessment Service (RAS), undertaking over the phone, and face to face assessment, underpinned by a wellness approach to enable clients to continue to live independently in the community of their choice. Your day to day will see you undertake screening and assessment in accordance with the relevant programs, ensuring a holistic approach focusing on the client strengths and abilities in order to maximize benefits, and optimize health outcomes. We are seeking someone with an exceptional telephone etiquette and extraordinary customer service skills to provide exemplary service delivery to both internal and external customers. We need someone who will approach each engagement in an empathetic, friendly, and patient manner - building rapport, trust and ensuring a positive and professional manner.Your typical day will see you:Undertake assessments as scheduled to ensure assessment activity timeframes, including response times are compliant with operational requirementsComplete support plan reviews and initiate linking support activities as identifiedIn consultation with the client, utilise the recommendations generated in the (VHC View) or (NSAF) to develop an individualised Support plan promoting independent living as practicable and self-reliant as possibleWe need you to have/be:Relevant qualifications and/or 3 years’ experience working in Health and/or Aged Care environmentDemonstrated experience in aged care assessments and service coordinationHigh level computer literacy, including proficiency with MS Office programs particularly Outlook, Word and Excel Well-developed communication skills, the ability to work independently, and be resilient in an ever-changing environmentUnrestricted Australian Driver’s LicenceThis is a fixed term role until 30/06/2022 (with the possibility of renewal), which offers an exciting opportunity to be trained in the RAS space.
