Clinical Program Coordinator, Virtual Social Centre
As Clinical Program Coordinator, Virtual Social Centre, you’ll be responsible for the clinical oversight and creation, development, coordination, of the relevant Virtual Social Centre (VSC) programs. Feros Care’s Virtual Social Centre (VSC) is an innovative virtual space where clients can actively participate in a range of live and recorded events covering diverse interests from exercise and movement to book clubs, languages, cooking and healthy eating, bingo, trivia, singing, and coffee catch ups - all from the comfort of their own home. Responsibilities in this role include: Building and maintaining sustainable virtual programs across all domains of the Feros Care Wellness Model to sustain or improve health outcomes and foster social connection Support and management of program hosts, both contractors and volunteers Ensure clinical specifications for all programs across the VSC are complete and up-to-date Aligning individual participant goals to Allied Health clinical programs Aligning programs to relevant funding, clinical models of care and the Aged Care Single Quality Framework Additionally, the role serves as the clinical point-of-contact for the VSC with referring clinicians (both internal and external) in collaboration with the Community Coordinator to facilitate appropriate and safe care for participants with an identified clinical need, planned to be provided by the VSC (e.g. group exercise programs) Essential Criteria: Relevant industry knowledge/the ability to rapidly acquire knowledge of Commonwealth-funded community-based Aged Care services, Residential Aged Care services or other community health care or disability services Experience in managing and leading staff and contractors (paid or volunteer) Experience in program design for clinical and non-clinical programs Demonstrated experience in care plan development for consumers in a goal-oriented, wellness and reablement context with a focus on self-determination and self-management Ability to demonstrate an understanding of the factors contributing to social isolation or risk of social isolation and the methods to prevent and address poor health outcomes through social prescribing methodologies An understanding of Healthy Community Indicators and how these can be applied in a virtual setting Qualifications, Certificates and Registrations: A relevant tertiary qualification in Allied Health or a related discipline including current registration with AHPRA, ESSA or AASW as appropriate.
