We have an exciting opportunity for a Registered Nurse to join the team as a Clinical Multi Program Assessor with Aspire4Life. This role will see you work within our Veterans Home Care (VHC) team on behalf of the Department of Veterans Affairs (DVA), and use your clinical skills to support the assessors in reviewing and approving care plans (these support plans are underpinned by a wellness approach to enable clients to continue to live independently in the community of their choice). In addition, you will support the team with virtual assessments during high-volume periods and also have the opportunity to move into other clinical ad hoc projects, including the Regional Assessment Service (RAS). This role offers fantastic flexibility, with the opportunity to be office based (Coolangatta), or work predominately from your home. We offer comprehensive training in this role, so we would need to see you in the office for the first couple of weeks (and we would also love to see you visit a couple times each month…) In terms of working hours, we are keen to speak with candidates who can work full time hours, but also open to this role on a part time basis - 0.6 or 0.8 fortnight is certainly an option!A typical day will see you:Review and approve care plans generated by our Veteran Home Care and Multi Program assessors, via the relevant department portalEnsuring a holistic approach, focusing on the client strengths and abilities is undertaken in each assessment, linking in and referring appropriate supports and servicesIn consultation with the client, utilize the recommendations generated in the assessment to put together an individualized support plan (promoting independent living as practicable and self-reliant as possible)We are seeking someone who is self-motivated and can confidently work autonomously, with a logical approach to problem solving and juggling competing priorities.
