This is an exciting opportunity to be working with the Home Care Packages (HCP) and Short Term Restorative Care (STRC) programs, applying your case management skills to provide holistic person-centred approaches to care planning and coordination of services.You will split your time between working from home and within the Grampians community to support your clients on their wellbeing journey.
