Clinical and Home Support Assessor – South Coast
Due to an internal promotion, we have an exciting opportunity to join the Gold Coast Regional Assessment Service with Aspire4Life as a Clinical and Home Support Assessor, on a full time basis. In this role, you will conduct assessments using a holistic and client centred approach, with a focus on wellness and reablement. These assessments will support older Australians to continue living independently within their chosen community, by considering needs such as assistive technology, social support services, and community care. You will have the opportunity to travel throughout the Gold Coast (North to South) to work collaboratively with clients in their homes (candidates will preferably need to reside in the central Gold Coast area). This position will commence in mid May, on a fixed term contract until 18 April 2022 (with the possibility of extension). This position offers a fantastic 2 weeks of full-time training and orientation at our head office in Coolangatta, QLD (all expenses covered), commencing in mid May 2021. What will I be doing as a home support assessor?Community based role visiting older Australian’s in their homes - you will drive throughout your broader region with a rostering team determining the best run to your first client and home from your last clientConduct holistic assessments with a focus on wellness and reablement and a client centred approachCapturing vital client information to build on the My Aged Care client storyDeveloping support plans and identifying unique individual goalsConnecting people with appropriate services and supports in their community. This may include assistive technology, social support or community careNeed to be/have:Knowledge of assessment and goal orientated support planning in aged, community, health or social servicesPrevious experience working in aged care, community services, assessment and/or coordination of servicesConfident communication and interpersonal skills, with the ability to engage and motivate clients, families, staff and other stakeholders with a positive approachSome local knowledge and/or the ability to network with community aged care service providers and stakeholder within your regionComfortable travelling long distances by carWhat to bring?Tertiary qualification as a Registered Nurse, Social Work, Occupational Therapist or PhysiotherapistCurrent AHPRA RegistrationUnrestricted Australian driver's licence and own reliable insured vehicleAbout usWe want our team to be made up of people from all walks of life.
